If Anthony looks female, people are going to slip up from time to time even if that hurts him. If it's done repeatedly by the same person, that's a problem. But maybe Anthony could have spoken to the manager first before having a go at the customer? In Australia, we don't really call people "sir", "ma'am" or "miss" - just "you". So that's another way to get around the issue :)
